Your Smile Savings Plan benefits are available only at Crabtree Valley Dental.
Our basic plan for $495 will include the following:
• 1 Comprehensive /New Patient Exam
• 2 Annual Exams
• 1 Emergency Exam (used any time during the year)
• 2 Cleanings (non-perio based or 2 routine perio)
• 1 Any Individual X-rays needed throughout the year
• 2 Fluoride Treatments (per year)
• 2 Bitewing X-rays (one set per year)
• Panorex and Full Mouth Series of X-rays
• 1 Emergency Exam (used any time during the year)
• 20% OFF Additional Services

Dental Savings Plan Program Guidelines
- Payment in full is due at the time of service
- 5% OFF of annual premium if plan is renewed prior to expiration date
- Cannot be used in conjunction with another dental plan or financing program such as Care Credit
- Premiums are non-refundable

** The family plan includes family members and children under 18 or children who are enrolled in college full-time until the age of 26.

Program Exclusions & Limitations
This program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan, dental insurance, or financing program such as Care Credit.
- For referrals to specialists deemed necessary by our doctors.
- For hospitalization or hospital charges of any kind.
- For treatment of injuries covered under workers’ compensation.
